Calprotectina fecal e elastase-1 fecal como marcadores inflamatórios intestinais e de função pancreática na fibrose cística

Abstract

Resumo: A Fibrose Cística (FC) é uma doença genética de maior prevalência entre euro descendentes. As mutações afetam o gene CFTR que codifica a proteína de mesmo nome, reguladora de água e cloro na membrana celular, ocasionando os sintomas multissistêmicos da doença. Os principais sistemas acometidos são o trato respiratório e gastrintestinal, sendo as inflamações gastrintestinais recorrentes nos pacientes. Neste contexto, testes diagnósticos de inflamação são úteis. Dentre eles, incluem-se Calprotectina fecal e Elastase-1 fecal, marcadores do processo de inflamação intestinal e função pancreática, com alta sensibilidade e especificidade. O objetivo desse estudo foi avaliar os níveis de Calprotectina (CLP) e Elastase-1 fecal em pacientes com FC. Foram coletadas amostras de 35 participantes, diagnosticados com FC e acompanhados em ambulatórios de FC do Complexo do Hospital de Clínicas da Universidade Federal do Paraná. Os pacientes com menos de 4 anos apresentaram níveis alterados de CLP, enquanto os saudáveis apresentaram níveis normais. Os pacientes acima de 4 anos apresentaram níveis mais alterados de CLP em presença de outras manifestações. Não houve significância nas associações entre valores alterados de CLP e manifestações clínicas da doença, bem como uso de antibióticos e enzimas. Os níveis de elastase fecal-1 estavam alterados em 27 de 35 pacientes, sendo apenas um considerado IP moderada. A CLP fecal e EL-1 fecal foram consideradas marcadores importantes na FC para acompanhamento da evolução da doença.

Abstract: Cystic Fibrosis (CF) is a genetic disease with highest prevalence among Euro descendants. The mutations affect the CFTR gene that encodes the protein of the same name, which regulates water and chlorine in the cell membrane, causing the multisystemic symptoms of the disease. The main systems affected are the respiratory and gastrointestinal tract, with recurrent gastrointestinal inflammation in patients. In this context, diagnostic tests for inflammation are useful. These include fecal Calprotectin and fecal Elastase-1, markers of the intestinal inflammation process and pancreatic function, with high sensitivity and specificity. The aim of the present study was to evaluate fecal Calprotectin and Elastase-1 levels in CF patients. Samples were collected from 35 participants, diagnosed with CF and followed up in CF outpatient clinics at the Complexo do Hospital de Clínicas of the Federal University of Paraná. Patients younger than 4 years old had altered levels of CLP, while healthy ones had normal levels. Patients older than 4 years had more altered of CLP levels in the presence of other manifestations. There was no significant association between altered CLP values and clinical manifestations of the disease, as well as the use of antibiotics and enzymes. Fecal elastase-1 levels were altered in 27 out of 35 patients, with only one being considered moderate PI. Fecal CLP and fecal EL-1 were considered relevant markers in CF for monitoring disease progression.
